Forms
All forms are posted on the Elections Website in accordance with the Calendar & Timeline section of the Elections Code. All forms are due no later than the deadlines set by the Elections Committee in accordance with the Calendar & Timeline section of the Elections Code. Interest and Volunteer Form
The Interest Form is used to collect information on people interested in the elections process and those who would like to volunteer. Emails and other information will be used from this form for the dissemination of information from the Elections Committee and the Elections Officer.
Intention to Run Form
The Intention to Run Form is used to collect information on candidates. Emails and other information will be used from this form for the dissemination of information from the Elections Committee and the Elections Officer and will be provided to bodies that request it for endorsement purposes if the candidate opts in on the form.
Violation Report Form
The Violation Report Form is for reporting potential violations of the Elections Code. If you suspect a Violation is taking place, please take photos or videos of the offence and submit them here. If you have questions on what constitutes a violation of the Elections Code, please refer to the section of the Elections Code on violation points.
Slate Declaration and Slate Alteration Forms
The Slate Declaration and Alteration Forms allows candidates to form and edit the membership of Slates. Slates exist for one election cycle and dissolve after the end of the election. Slates may contain 3 candidates for the ASUCD Senate and are organized by a Slate manager who may or may not be a candidate on the slate. Expenditure Form
The Expenditure Form allows the Elections Officer and the Elections Committee to verify that a candidate or ballot measure sponsor has not violated the mandatory Campaign Spending Agreement. Candidates for the ASUCD Senate may spend no more than $250 on their campaign. Ballot measure sponsors may spend no more than $250 on their campaign. The Expenditure Form must include both donations and expenditures for a campaign. An Expenditure form will be issued to each candidate after their completion of the Mandatory Spending Agreement. Endorsement Form
The Endorsement Form allows Registered Student Organizations (RSOs) or individuals who are members of the ASUCD to make endorsements for candidates for office or ballot measures. Endorsements are regularly posted on the Elections website after being verified by the Elections Officer. Candidate Workshop Excuse Form
The Mandatory Candidate Workshop is mandatory, as defined in the Elections Code. Excuses for absences must be approved by the Elections Committee. The Mandatory Candidate Workshop will be held at a to be determined date, time and location.
Voter Guide Statement Form
The Voter Guide Statement Form collects the statements of candidate which will then be presented in the voter guide. The voter guide will be sent to various organizations around campus to help endorse the Elections as a whole.
Debate Question Form
The Debate Question Form allows members of the public to submit questions to be asked to candidates for the ASUCD Senate at the debate. the Debate for ASUCD Senate will be held at a to be determined date, time, and location.
